Position Summary: The Manager, Patient Safety & Clinical Risk reports to the Director and manages the Patient Safety and Risk program for the medical group. Provides input and support for development of annual site specific Patient Safety Plan. Coordinates patient safety improvement programs, including Safety Champion and Active Disclosure Team, that reduce the risk of injury and promote reporting of events.  Develops and delivers education programs based on event reporting data in collaboration with staff development.  Administers the Safety First Program, including but not limited to the investigations and Root Cause Analysis. 

 

Essential Functions and Responsibilities:

 

  1. Provides management for patient safety including:
    • Prevention, Detection and Correction of Patient Safety events
    • Staff education in error reduction
    • Culture of Safety
    • Just Culture
    • Annual Risk Assessment of Medical Group
    • Site visits and Practice Risk Assessments
  1. Assists in the development and implementation of Patient Safety Annual Goals and Objectives in line with McLaren and MMG Strategic Plan
  2. Assists with the development of the Patient Safety Plan and Areas of Focus
  3. Patient Safety Identification/Event Management
    • Reviews incident trends for patients, employees and visitors and makes recommendations for case status
    • Identify adverse trends or individual patient safety concerns through Safety First, chart abstraction, referrals from personnel, and any others identified.
    • Notifies administration of quality or regulatory issues as appropriate when negative trends or events occur which need intervention and monitoring.
    • Supports in the identification and aggregation of trends, including adverse trends, recommendations and/or patient safety concerns for presentation to CMO and Patient First Committee
    • Compiles information from staff, managers, site administrators and physicians on patient safety concerns and provides recommendations to leadership for next steps
    • Conducts initial investigation of all potential patient safety events and provides summaries and recommendation to director and site administration ongoing.
    • Investigations include but are not limited to internal and external medical record review, staff and witness interviews, and other communication as appropriate
    • Facilitates the Root Cause Analysis process in conjunction with Director & CMO
    • Works independently to promote patient safety to clinics, offices and providers, utilizing culture of safety information.
    • Coordinates and presents education programs and other educational materials addressing areas of concern in patient safety
    • Provides risk and patient safety orientation to newly hired employees per MMG Staff Development
    • Provides consultation regarding crisis situations (sentinel events, medical events, internal disaster, external disaster, etc.)
    • Acts as system resource for disclosure process and learnings

 

  1. Proactively manages patient safety utilizing data, information from Patient Safety Committees, Executive Rounding and other reports to analyze issues and determine their root causes.
  2. Collaborates with clinical leadership to develop strategies to minimize incidents. Develops programs and services that produce measurable improvements processes and practices that affect patient safety.
  3. Serves as a liaison to ensure clinical services are provided in accordance with safety standards established by federal and state regulatory agencies and the Joint Commission accreditation standards, including the National Patient Safety Goals.
  4. Communicates with and educates leadership and staff regarding patient safety initiatives and activities.
  5. Performs related duties as required.

 

Required:
  • Bachelor’s Degree in healthcare field
  • Certified Professional in Healthcare Risk Management (CPHRM) or other Patient Safety training/certification desired (CPPS, IHI Executive Development) within 6 months of hire
  • Minimum of five years of healthcare experience, preferably in a risk and patient safety management role.
  • Current Michigan Driver's license and proof of valid automobile insurance. 
Preferred: 

Current Certified Professional in Healthcare Risk Management (CPHRM) or other Patient Safety training/certification desired (CPPS, IHI Executive Development).

Clinical Bachelor’s Degree
