Clinical Support Specialist
1. Assists Population Health Supervisor in supporting all aspects of department operations, compliance, and reporting.
2. Performs Care Coordination services for MPP including but not limited to the use of ADT systems, population health tools, patient outreach, and communication across the care continuum with physician offices, home care providers, hospitals, skilled nursing facilities, and insurance companies.
3. Serves as the Lead Population Health Navigator which includes supporting daily operations, mentoring new staff, and collecting quality and compliance data for the purpose of process improvement.
4. Performs lead trainer duties for Population Health processes and Population Health Navigation.
5. Serves as a super-user for MPP Information Systems including but not limited to, all Population Health software and tools, provider databases, and other software applications.
6. Responsible for tracking and compiling data reports to support department operations, individual practices, and other McLaren subsidiaries participating in value-based care transformation. Examples include but not limited to: ADT activity and utilization, TOC appointment eligibility and scheduling, all billing reports and all other care coordination related reporting as needed.
Required:
• Associate degree in healthcare or related field.
• Three (3) years of ambulatory practice experience
• Two years’ experience in Population Health Industry
Preferred:
• Bachelor’s Degree in healthcare or related field.
• Two years’ experience in a health plan or Physician Organization environment with Care Coordination, Utilization Management, disease management, and/or population health.
