Position Summaries:

Home Health:

Responsible for the management of cost-efficient care to home health patients ensuring quality delivery of outcomes based on care that meets the individual needs of the patients/families.  Responsible for identification of educational opportunities from performance improvement and record review activities to ensure quality processes and outcomes are implemented in the Home Care Quality Plan for the Program. 

Hospice:

Is responsible to oversee the daily operations, programs and services of the nursing and home health aide departments.  To assure quality patient services and facilitate processes in accordance with agency policies and procedures.  Plans, directs, and participates in the delivery of professional services, assists in controlling costs through effective use of personnel, establishes and implements priorities for services.

Essential Functions and Responsibilities:

Home Health:

1.    Assists with evaluation of the clinical team and provides on-the-job training in deficient areas.
2.    Evaluates the provision of patient care and accuracy of supporting documentation through patient clinical record audits including but not limited to; OASIS, ICD-10 coding, Plans of Care, initial physician orders.
3.    Monitors staff performance related to OASIS documentation.
4.    Assists, prepares, and presents at operations meetings, and submits required reports in a professional and timely manner.
5.    Assumes on call responsibilities on a rotational basis.
6.    Translates and interprets policies and procedures that guide and support the provision of services of the agency.

Hospice:

1.    Supervises and evaluates assigned staff and provides on-the-job training, assuring clinical competencies.
2.    Explains and presents information regarding performance issues to staff.
3.    Communicates effectively with staff, clients, families and other disciplines.
4.    Interacts with health care professionals and physicians.  
5.    Provides customer service consistent with organization values, standards, and policies.
6.    Oversees the quality and appropriateness of services and staffing as well as their availability to meet patient care needs.

Home Health:

Required:

•    Current license to practice as a Registered Nurse in the State of Michigan    
•    Minimum of two years home health care experience
•    Valid driver’s license, proof of valid auto insurance, and maintain a driving record in accordance with McLaren’s corporate driving policy and eligibility guidelines
•    BLS certification required within first week of hire.

Preferred:  

•    Bachelor of Science in Nursing Degree (BSN)
•    One year of management experience
•    Experience with homecare regulatory and accrediting bodies

Hospice:

Required:

•    Current license to practice as a Registered Nurse in the State of Michigan.    
•    Two years progressive supervisory experience.
•    One year experience in a Hospice setting.
•    Valid driver’s license, proof of valid auto insurance, and maintain a driving record in accordance with McLaren’s corporate driving policy and eligibility guidelines
•    BLS certification required within first week of hire.

Preferred:  

•    Bachelor of Science in Nursing Degree (BSN)
