Qualifications:
Required:
•    Graduate of an accredited school of nursing with MSN
•    Current license to practice as a Nurse Practitioner in the State of Michigan.
•    Minimum of one to two years of med/surg, critical care or demonstrated experience in a specialty area.  
•    Current Michigan driver’s license and proof of valid automobile insurance.

Preferred:  
•    Two years hospital experience.
•    One year of hospice care experience.


Functions as Primary Nurse in assigned geographical area:

•    Responsible for delivery of care to patients within area.
•    Prepares a daily schedule of planned visits and establishes priorities on a weekly basis to assure physician ordered frequencies are met.
•    Provides teaching of special procedures as needed.
•    Prepares Home Health Aide plan of care form for each patient and makes supervisory visit to HHA’s patients every two weeks.
•    Uses Medical Social Worker, Dietitian and therapists as resources and advises attending physician of patient’s need for other services when indicated.
•    Confers with other health care personnel in planning, implementing and evaluating service to coordinate patient care.
•    Responsible for the timely preparation of closing summaries to physicians and renewal of physician’s plan of treatment every 60 days with direction from Clinical Services Supervisor.
•    Other duties as requested.
