Position Summary:

Ensures the overall administration and implementation of bereavement services provided while adhering to Medicare and state-specific regulations. Provides assistance with issues related to grief and loss, as well as spiritual support and any other counseling services for the client and family in keeping with their desires and belief system.

Essential Functions and Responsibilities:

1.    Develops a bereavement plan of care that notes the kind of bereavement services to be offered and the frequency of service delivery. 
2.    Provides bereavement services including spiritual support and any other counseling services for the client and family consistent with the bereavement plan of care. 
3.    Coordinates the assessment and delivery of bereavement and grief counseling needs and services (via phone, office visits, home visits, groups and ongoing follow-up) to ensure timely and appropriate services are provided to client/family/caregivers. 
4.    Participates in interdisciplinary group meetings and actively communicates with nursing and other team disciplines. 
5.    Provides consultation, education, outreach, bereavement information, and referral to hospice team members, other professionals, and the community at large. 
6.    Collaborates with other interdisciplinary team members to promote ease of transition from patient care to bereavement services. 

Required:

•    Bachelor’s degree from an accredited school in social work, psychology, counseling, pastoral education, or related field. 
•    Six months experience in bereavement programs or support activities.
•    Current state driver’s license and proof of valid automobile insurance

Preferred:  

•    Experience providing support to clients and families dealing with life threatening illness or death.

Required:

•    Valid driver’s license, proof of valid auto insurance, and maintain a driving record in accordance with McLaren’s corporate driving policy and eligibility guidelines.
