Central Intake Clinical Specialist
- Responsible for coordination of agency referrals and inquires, data entry of all clinical referral information, access and review of MHC hospital EMR to gather appropriate clinical information, documentation and processing of all paperwork for patient care services, and for the accuracy, timeliness and detail of referrals
- Provides for continuity of care for all clients admitted to the agency by coordination with hospital discharge planners, physicians, client/families, Mclaren Visiting Nurse & Hospice staff and agencies to which clients are referred.
- Provides information regarding agency services/programs and community services available that may meet client/family needs.
- RN or LPN Graduate of an accredited school of Nursing
- Current License to practice as a RN in the state of Michigan or current License to practice in the State of Michigan with NAPNES
- 2 years Med/Surg experience.
- Ability to communicate effectively, as a liaison to primary referral source, families, team members, and community agencies.
- Demonstrate a high level of skill in problem solving and working effectively with people.
- Must be detailed orientated with ability to coordinate multiple tasks simultaneously.
- 1 year Home Care and/or Hospice experience.
- Managed Care experience.
- 1 year Discharge Planning and/or Utilization Review experience.
